Renewal House is seeking a full-time Case Manager to provide to clients in the Family Residential Program. The Case Manager will be responsible for providing effective individual case management for mothers in recovery, including care coordination with community partners. This position also provides weekly therapeutic groups for clients. The candidate will be committed to providing high quality case management services in a trauma-informed manner to clients with substance-use disorders and co-occurring mental health disorders. Renewal House is a comprehensive addiction recovery community for women and their children, which includes family residential services for pregnant and parenting women and licensed intensive outpatient and recovery support services for women.
Job Responsibilities
The Case Manager, who reports to the Program Services Manager, is responsible for the following:
Provide case management service to clients on caseload.
Facilitate weekly therapeutic groups.
Role-model the skills needed for successful social and business interactions.
With other clinical team members and the individual client, create individual treatment plans and behavioral modification plans for clients as needed.
Participate actively in weekly clinical team meetings and other staff trainings.
Complete documentation in a timely fashion that meets quality standards.
Required Qualifications
We’re seeking candidates who excel at financial management, are results oriented, and have strong accounting skills. Candidates should have:
Strong written and verbal communication skills.
Communication skills and knowledge normally associated with a master’s degree in a related field.
Experience working with substance abuse and/or co-occurring mental health disorders.
Ability to effectively use technology to enter and obtain data.
